We know how important it is for hospital discharge planning and utilization review staff to free up needed hospital beds. Further, the discharge of a patient to hospice services is stressful for hospital staff, patients, and family members. Our promise is to provide immediate response to your patient referrals.
- Our goal is to make contact with the patient and family within the hour of your call to our referral office.
- Our goal is to assess the appropriateness of each referral and admit appropriate patients within three (3) hours on the same day they were referred.

We know that your physicians like to participate in the development of their patient's Plan of Care and be kept informed of any changes in their patient's condition.
- Our Goal is to involve physicians on your staff in the development of your patient's Hospice Plan of Care.
- Our Goal is to provide careful follow-up communication with physicians on your staff using the communication protocols we have established with the physicians on your hospital staff.

We know that a smooth transition from hospital to home is your commitment to your patients. Our Admission Staff are exceptionally good at anticipating what is required for a smooth transition for each patient that we accept into our program.
- Our Goal is to use our best efforts to “handle the paperwork” and expedite the transition.
- Our Goal is to ensure that all aspects of the transition are coordinated so that the patient gets home after Durable Medical Equipment and Medical Supplies have been delivered to the home.
