How to Work With Hospice: Physician Providers

Our Service Promises to Physicians

Hospice of Arizona's service promises to physicians set it apart as the hospice of first referral choice among community physicians.
  1. We know how anxious your patients and their loved ones are when you recommend hospice services for the first time. Our promise is to be attentive and responsive to your patient referrals.
    1. Our goal is to make contact with the patient and family within the hour of your call to our referral office.
    2. Our goal is to assess the appropriateness of each referral and admit appropriate patients the same day they were referred.

  2. We know that physicians like to participate in the development of their patient's Plan of Care and be kept informed of any changes in their patient's condition.
    1. Our Goal is to involve you in the development of your patient's Hospice Plan of Care.
    2. Our Goal is to report on your patient's condition based upon your requirements using the communication protocols we have established with your office.


Attending Physician's Role in Hospice
  • Certify Patient Eligibility (Patient's prognosis can be spoken of in terms of months not years)
  • Approve the Hospice Admission
  • Determine their own level of involvement in the provision of care
  • Approve all services to be provided
  • Receive and review regular progress notes
  • Review adequacy and appropriateness of Hospice services
  • Inform Hospice of any significant changes in the patient or family status
  • Receive reimbursement for all services

Receive Care Plan Oversight payments for Medicare Payments
(See Reimbursement for Details)

The Centers for Medicare and Medicaid Services (CMS) provides “Care Plan Oversight” reimbursement to compensate you for the time you spend documenting and treating your hospice patients. This benefit is paid for each hospice patient that you refer to hospice. You must document at least 30 minutes of Care Plan Oversight time each month to be eligible for this reimbursement.
